Normal Variations in Labor and the First Days

Birth and the first days come with a lot of new sensations, sights, and behaviors. Many of them are startling the first time and completely expected. Others look similar but do need a call. This topic collects the most common examples so you can recognize them — and, importantly, so you know which ones deserve a call to your care team anyway.

• Braxton-Hicks / prodromal contractions

  Within the range of normal

  Irregular, painless-to-uncomfortable tightening that comes and goes for hours or days, often stronger in the evening. They soften the cervix but don't dilate it in a steady pattern.

  Call your healthcare team if

  • Contractions become regular — roughly every 5 minutes for 1 hour
  • You can't talk through the contractions anymore
  • Your water breaks (a gush or slow trickle)
  • Vaginal bleeding heavier than a light pink streak

  Get emergency care now (call 911)

  • Sudden severe belly pain that doesn't ease between contractions
  • Heavy bright-red bleeding, especially with pain (possible abruption)

• Bloody show / mucus plug

  Within the range of normal

  Pink, brown, or blood-tinged mucus discharge in the days or hours before labor as the cervix softens and opens. Can happen once or off-and-on over several days.

  Call your healthcare team if

  • You soak a pad with bright red blood
  • You pass clots larger than a quarter
  • The bleeding is heavier than a period

  Get emergency care now (call 911)

  • Sudden gushing bright-red bleeding with belly pain (possible abruption)
  • You feel faint, dizzy, or short of breath

• Water breaking without contractions (PROM)

  Within the range of normal

  A gush or slow trickle of clear or pale-yellow fluid. Most people begin labor within 24 hours on their own.

  Call your healthcare team if

  • Fluid is green, brown, or foul-smelling (possible meconium or infection)
  • You have a fever (≥100.4°F / 38°C)
  • The baby is moving less than usual
  • You're less than 37 weeks pregnant

  Get emergency care now (call 911)

  • You feel a cord or a bulge in the vagina after the water breaks (possible cord prolapse — call 911, get onto hands-and-knees)
  • Heavy bright-red bleeding with the fluid

• Reduced fetal movement late in pregnancy

  Within the range of normal

  The type of movement changes as the baby runs out of room — more rolls and stretches, fewer big kicks. You should still feel regular movement every day.

  Call your healthcare team if

  • A clear decrease from your baby's usual pattern
  • Fewer than 10 movements in 2 hours after eating and lying on your side
  • Any day with no movement at all — call the same day, don't wait for tomorrow

  Get emergency care now (call 911)

  • No movement at all after a cold drink, snack, and 30–60 minutes of lying on your side — go to labor & delivery immediately

• Long latent phase (early labor)

  Within the range of normal

  Early labor can last hours to a couple of days, especially for first babies. Contractions may space out at night, then pick up again. Eating, hydrating, resting, and distraction are the plan.

  Call your healthcare team if

  • Contractions are 3–5 minutes apart and painful for 1+ hour
  • You've been awake all night unable to rest
  • Your water breaks
  • Bleeding is heavier than mucus-plug show
  • Your provider gave you a specific 'come in' trigger and you've hit it

• Back labor

  Within the range of normal

  Intense pain in the low back, often from a baby facing forward (OP) or pressing on the sacrum. Counter-pressure, hands-and-knees, side-lying release, and hip squeezes usually help.

  Call your healthcare team if

  • Pain is constant between contractions and severe (not just intense during a wave)
  • Any bleeding heavier than show
  • You're worried the baby's not moving

  Get emergency care now (call 911)

  • Sudden severe belly pain with no relief between contractions plus bleeding (possible abruption)

• Shaking, teeth-chattering, vomiting in transition

  Within the range of normal

  A hormone surge as labor ramps up — shaking, sudden nausea or vomiting, hot/cold flashes, feeling like you can't do this. Often means you're close.

  Call your healthcare team if

  • Shaking with a fever ≥100.4°F (possible infection)
  • A bad headache or vision changes (possible preeclampsia)
  • Upper-belly pain or pain under the right ribs (possible preeclampsia / HELLP)
  • Swelling of face or hands that's new or worse

  Get emergency care now (call 911)

  • Chest pain or trouble breathing — call 911
  • A seizure — call 911

• The "purple line" up the buttock crease

  Within the range of normal

  A purple-red line that creeps from the anus up the crease as labor progresses — thought to reflect pelvic pressure. It's a bonus data point, not a required one.

  Call your healthcare team if

  • You wouldn't call about the line itself — call about anything else in this list that appears alongside it

• "Fetal heart rate variability" on the monitor

  Within the range of normal

  A jagged, up-and-down baseline (moderate variability, 6–25 bpm) is a reassuring sign the baby is well-oxygenated. Brief accelerations with movement are also reassuring.

  Call your healthcare team if

  • You're told the tracing has changed category (from I to II or III)
  • Repeated late or deep variable decelerations that aren't recovering
  • You want a plain-English explanation of what the strip shows — always ask

• Swollen cervical lip / "anterior lip"

  Within the range of normal

  A small strip of cervix that hasn't fully retracted at 9–10 cm. Common. Position changes, waiting, or manual reduction usually resolve it.

  Call your healthcare team if

  • You wouldn't call about this — your team will name it during an exam
  • Ask what your options are (wait, position change, help it over) before pushing against a lip

• Feeling the urge to push before you're "complete"

  Within the range of normal

  Some people feel pressure and grunty urges before 10 cm — often means the baby is descending. Some teams will let you follow gentle urges; others prefer you wait.

  Call your healthcare team if

  • Flag it to your provider right away so they can check whether you're complete
  • Ask whether gentle bearing-down (versus holding back) is safe given your exam

• Pushing for 2–3+ hours

  Within the range of normal

  First-time parents commonly push 2–3 hours, and with an epidural it can be longer as long as the baby is descending and tolerating it well. ACOG says at least 3 hours (first baby) or 2 hours (subsequent) is normal before calling second stage "prolonged."

  Call your healthcare team if

  • Your provider tells you the baby isn't tolerating pushing (heart-rate concerns)
  • You're exhausted and want to discuss position changes, rest, or assisted birth
  • No descent after clear rest and position changes

• Meconium in the water

  Within the range of normal

  Green- or brown-tinged fluid — the baby had a bowel movement in utero. Often just a sign of a mature or a slightly stressed baby; most babies do fine.

  Call your healthcare team if

  • Any meconium — report to your team on arrival
  • Water breaks at home with green/brown fluid — go in promptly
  • Reduced fetal movement alongside meconium

  Get emergency care now (call 911)

  • Thick pea-soup meconium with a baby who's moving less — go to labor & delivery immediately

• Cone-shaped head, swelling, or bruising

  Within the range of normal

  Molding (a temporarily elongated head), caput (soft scalp swelling), and small forceps/vacuum bruises are common and resolve over days to a couple of weeks.

  Call your healthcare team if

  • Swelling is boggy, growing, or feels like a water balloon that shifts under your fingers
  • The baby is unusually sleepy or hard to wake for feeds
  • The baby seems in pain when the head is touched
  • Pale color or poor feeding after a vacuum or forceps birth

  Get emergency care now (call 911)

  • Baby is pale, floppy, or breathing hard after an assisted delivery — get emergency evaluation (possible subgaleal hemorrhage)

• Purple/blue hands and feet (acrocyanosis)

  Within the range of normal

  Bluish tint at the hands and feet in the first 24–48 hours as circulation matures — while the trunk and lips are pink.

  Call your healthcare team if

  • Fingers or toes stay blue past 48 hours
  • Baby feels unusually cool or floppy

  Get emergency care now (call 911)

  • Lips, tongue, or trunk look blue or gray (central cyanosis) — call 911
  • Grunting, flaring nostrils, or ribs pulling in with each breath — call 911

• Blotchy, peeling, or spotty newborn skin

  Within the range of normal

  Milia (tiny white bumps), erythema toxicum (red splotches with tiny white centers that migrate), Mongolian spots (blue-gray birthmarks), and post-dates peeling all resolve on their own.

  Call your healthcare team if

  • Blisters filled with clear or yellow fluid
  • Spreading redness or a rash that looks bruised
  • Any fever (rectal ≥100.4°F / 38°C in a baby under 3 months)

  Get emergency care now (call 911)

  • Fever ≥100.4°F in a newborn under 3 months — go to the ER now
  • Rash that doesn't blanch when you press on it (petechiae) — go to the ER now

• Newborn hiccups, sneezes, and "grunty" breathing

  Within the range of normal

  Newborns hiccup after feeds, sneeze to clear amniotic fluid from their nose, and have irregular breathing with brief pauses (up to about 10 seconds) — all normal in the first weeks.

  Call your healthcare team if

  • Breathing over 60/minute at rest, counted for a full minute
  • Grunting with every breath
  • Chest sinking in (retractions) between the ribs or under the ribcage

  Get emergency care now (call 911)

  • Breathing pauses longer than ~20 seconds
  • Blue color around the mouth, lips, or tongue — call 911
  • Baby is limp, unresponsive, or won't wake — call 911

• Swollen breasts or vaginal spotting in the newborn

  Within the range of normal

  "Witch's milk," a few days of tiny breast swelling in babies of any sex, and a small amount of pinkish vaginal discharge or spotting in newborn girls — all from maternal hormones and resolve in days to weeks.

  Call your healthcare team if

  • Breast area is red, hot, or hard (possible mastitis neonatorum)
  • Any fever ≥100.4°F in a baby under 3 months
  • Foul-smelling discharge

• 7–10% weight loss in the first days

  Within the range of normal

  Term babies commonly lose 5–7% of birth weight and are back to birth weight by ~10–14 days. Some fluid was IV, some is meconium output.

  Call your healthcare team if

  • Weight loss ≥10% of birth weight at any check
  • Baby isn't back to birth weight by 2 weeks
  • Diaper output is dropping (see next entry)

• Wet & dirty diaper count in the first week

  Within the range of normal

  Rule of thumb: day 1 = at least 1 wet / 1 stool, then add roughly one per day, reaching 6+ wets and 3+ yellow seedy stools per day by day 5–7.

  Call your healthcare team if

  • Fewer wet diapers than the day-of-life number in the first week
  • No stool in the first 24–48 hours
  • Brick-red or dark-orange 'brick dust' urine after day 3 (dehydration signal)

  Get emergency care now (call 911)

  • Baby is limp, hard to rouse, sunken soft spot, or dry mouth (dehydration) — go to the ER

• Cluster feeding and day-2 fussiness

  Within the range of normal

  Long stretches of near-constant feeding, especially in evenings and around day 2, day 10, and 3 weeks. It's how the baby drives up your milk supply — not a sign you don't have enough.

  Call your healthcare team if

  • Baby is sleepy and hard to wake for feeds
  • You can't hear or see swallowing during feeds
  • Diaper output has dropped below the expected day-of-life count
  • Weight loss trend is worsening at the weight check

• Mild newborn jaundice

  Within the range of normal

  Yellow tint appearing on the face after day 2 and moving down as bilirubin rises mildly — expected in about 60% of term babies.

  Call your healthcare team if

  • Yellow reaches the belly or legs (not just the face)
  • Eyes look deep yellow
  • Baby is very sleepy or feeding poorly
  • Jaundice appears in the first 24 hours — skip visual guessing and get a bilirubin level

  Get emergency care now (call 911)

  • Baby is limp, arching, or has a high-pitched cry (possible severe hyperbilirubinemia) — go to the ER

• Afterpains and heavy lochia

  Within the range of normal

  Cramping (especially with breastfeeding, and worse with each baby) for 2–3 days, and dark red bleeding heavier than a period for the first few days that gradually lightens and turns pink then yellow-white over 4–6 weeks.

  Call your healthcare team if

  • Soaking a full maxi pad in an hour
  • Passing a clot bigger than a lemon (or more than one egg-sized clot)
  • Bleeding suddenly turns bright red again after slowing down
  • Fever ≥100.4°F, chills, or foul-smelling discharge

  Get emergency care now (call 911)

  • Feeling dizzy, faint, or short of breath — call 911
  • Chest pain or leg swelling and pain (possible blood clot) — call 911
  • Sudden severe headache or vision changes (possible postpartum preeclampsia) — go to the ER

• "Baby blues" in the first two weeks

  Within the range of normal

  Tearfulness, mood swings, feeling overwhelmed, and trouble sleeping in the first 10–14 days — a hormonal shift most people experience.

  Call your healthcare team if

  • Symptoms last longer than 2 weeks
  • You can't sleep even when the baby sleeps
  • You feel disconnected from the baby or from yourself
  • You feel hopeless, worthless, or can't enjoy anything

  Get emergency care now (call 911)

  • Any thoughts of harming yourself or the baby — call 988 (Suicide & Crisis Lifeline) or 911 now
  • Hearing or seeing things others don't, or beliefs that feel unusually powerful (possible postpartum psychosis) — go to the ER

• Night sweats and postpartum shedding

  Within the range of normal

  Drenching sweats at night for a few weeks as your body offloads pregnancy fluid, plus dramatic hair shedding around 3–4 months postpartum. Both resolve.

  Call your healthcare team if

  • Sweats come with a fever ≥100.4°F or chills
  • Breast pain, redness, or a hard area (possible mastitis)
  • Foul-smelling vaginal discharge (possible endometritis)

  Get emergency care now (call 911)

  • One-sided leg swelling, redness, or calf pain (possible DVT) — call 911
  • Sudden shortness of breath or chest pain (possible PE) — call 911

Most of these are drawn from ACOG's postpartum care guidance, AAP newborn care resources, and the CDC's Hear Her campaign on urgent maternal warning signs. Common examples: baby heart-rate variability on the monitor; bloody show; water breaking without contractions; back labor; long latent phase; pushing for 2–3 hours (especially with epidural); a swollen cervical lip; the "purple line"; shaking or vomiting in transition; meconium in the fluid; a cone-shaped newborn head; 7–10% weight loss in the first days; cluster feeding; day-2 fussiness. Warning signs that DO need a call include: soaking a pad an hour, sudden severe headache or vision changes, chest pain or trouble breathing, thoughts of harming yourself or the baby, a baby with fewer than the expected wet diapers, a baby who is very hard to wake or feeding poorly, and a baby whose skin or eyes turn deep yellow.

For most topics, guidelines from ACOG, WHO, and other major bodies broadly agree on the core safety points (when to act in an emergency, informed-consent standards, monitoring baselines). Where reasonable people — and even guidelines — differ tends to be around thresholds (when to start or stop something), preferences (comfort, environment, support), and values (how you weigh small risks against benefits).
